Chiswick PCN hub

I have to take my uncle for his Covid injection Txt  says come Chiswick pcn  does anyone know where this is It’s not my local GpAs I am in his only bubble he is 86 years old

When I got a text for a blood test it said come to the Chiswick PCN hub which, when I phoned my doctor's surgery I was told it was the 'middle desk' at the Chiswick Health Centre. However, I then got another text the day before my appointment saying I should go to the Wellesley Road Health Centre. Nice they keep us on our toes!
